Got a small form we use to request new accounts needing access. The form has 3 functions New Request, Termination, or Add/Remove Applications. These are set at the top as a check box. What I want is, when someone clicks New Request, all the boxes that are not needed to be filled out either disappear or gray out.
I have been stuck on this for hours, trying different formulas from all over the web and nothing has worked so far.
