Watch UFC 41 Mir vs Abbott. Mir wins by toehold. It's only called a toehold because the top of the toes and instep is one of the leverage points. It really hurts the knee and ankle. In fact, I think I may have a miniscus tear in my knee as a result (only my prognosis. I am not a doctor.)
