Can the companies tell if it's prepaid or not?
Pre-paid failures may be due to lack of funds. Many offers charge a temporary monthly fee to check if you have enough funds. I know true.com does this. It is a 7-day trial but they charge $50 on your card, and a couple days later it's gone.

It's called a pre-authorization, it's not actually a charge, i.e. it doesn't increase your account balance but it DOES reduce your available funds/credit until the auth expires, which can take from a couple days to a couple weeks.
Lots of the opt-out subscription type sites will do this.

Pre-paid cards, when accepted, offer you some protection against fraudulent/unauthorized charges in that the damage you suffer is generally limited to your available balance. Most credit cards have 0% fraud liability, though...and (at least my Citi cards have) virtual account numbers. I use mine, and in 2+ years I've had very few problems (all were resolved either through the offer company or my credit card companys' resolution units). I wouldn't recommend using a debit card as this is a direct link to your bank account, and most banks offer very little protection against fraudulent/unauthorized charges.
Just be sure to keep track of what offers you're signing up for, and Google around for some background information on offers that seem iffy.
