Yes. It happens for any number of reasons; most likely, he created multiple accounts with Trainn or tried to do the same offer twice.

More then likely it means they have multiple accounts and have had their account locked.

Completing the same offer twice [i849b7bfa1e]usually[/i849b7bfa1e] does not result in the account being locked, the only action required to correct that is completing a unique offer.
If they have done so numerous times or the offer abuse is very evident, then it may be locked.
