W-8ben tax form from Canada

Hi

I just finished YPS34Free and they need a tax form (W-8ben) from me.

HOW DO I FILL THIS IN? I read the instructions PDF file and it left me more confused than ever.

This is what I have so far

For Part I
1. Full name
2. N/A
3. Ticked 'Individual' and nothing else
4. Full address
5. BLANK
6. BLANK
7. BLANK
8. BLANK

Part II
9. Ticked 9.a and typed "Canada" in the blank; nothing else is ticked or typed.
10. BLANK! I know I have to write something here, but what???

Part III
11. BLANK/not ticked

Part IV
On "Capacity in which acting", what should I write? "Self"? Why can't they just say relation to signatory or something less obscure?

3--- 4 are ok

5 --blank (if adress is same)
6 -- leave it blank
7 --- like 6
8--- like6

9 --- check a write CA
10 -- noothing, leave blank
11 --check

then sign D and yes in individual is ok (beneficial owner or something)

well I had to ask kala too...cause i couldnt figure it out. its much simpler than it looks )

Well, tax forms are usually simpler than they seem but if you make a small mistake, it can cost you. p

I hate CRA/IRS jargon.
