W-9 Questions

First, this ( http//www.irs.gov/pub/irs-pdf/fw9.pdf?portlet=3 )
is the correct form right?

Secondly, would anyone know offhand how submitting one of these would affect my student status, or does this form just take taxes from the reward and that's it? I'm completely tax illiterate.

The W9 form itself does nothing but reside in the files of whomever you send it to. )

A W9 is an official record of a taxpayer's identity. Businesses (in this case, freebie companies) must maintain it as part of their records, it is used as the official source of the data that is printed on your 1099-MISC that you will get at tax time. A W9 is never sent to the IRS.

Oh, and yes, that is the correct form.

Not the W-9. You should get those from freebie sites before you receive a gift of $600 or more. You don't see that again and Prizebook just pin it to their wall outside their dormroom.
