did you actually have to ask him, or did he just send those out to you?
Yea, I got that message about 2 weeks ago. I posted a customer inquiry about it.

I had sent him a PM wondering why the approval process had taken 6 weeks up to that point... we're now approaching week 10....

I'm in the same boat- i sent my form for premiumipods in in mid march, and NOTHING at all has changed.
