I sent in the form, got put on hold, do i send it again?

In mid march I sent in my approval form for Premiumipods, but then got put on hold in April. I got it all sorted out, and am not on hold anymore, but do I need to send in the form again? WOuld they have just thrown it away once I was put on hold?

I would send it again just to make sure.
They have to re-review your account.
