I sent my approval form certified mail. I got delivery confirmation on May 18th. I got this card in the mail today. I can't read who signed for it. I guess that means that they do in fact have the form. They said two days ago that they didn't and I guess Jake is on vacation until Tuesday (read in a nother post). Anybody else have this person sign for their delivery? Maybe it is the mail carrier who signed that they delivered it. I don't know how they have someone waiting at a PO Box to sign for delivery.
