Flatscreens form sent, 1 referral denied, now send another?

I sent my flatscreen form in on 3/15, and had a referral denied on 4/23 (or around).

I just got another referral and have my magic 8 again. Do I need to send another form, or contact customer service (I couldn't even find the link).
