I have been approved for the PS3 site since 1/18/06. When I go to my status page I see that my status has been changed to "pending approval" I however do not see where to download the form.
Jake or any one in this situation. Do I need to send in a form also? If so where do I get it?

If you don't see the form you don't need to send it. The form is just to be approved, if you already were approved you don't need to send it.
For those who have a glitch - ie sites that went from processing to pending approval and it asks you to send in the form you do not have to do so, but I would send a support ticket to their CS anyway just so it is in writing from them that you do not.

Seems like this is just another chance to mess things up majorly for us not to finish these sites
i have the same question for my black 30gb ipod. cs hasnt answered it yet.

The vouchers are just for getting approved. We have already been approved so we dont have to worry.
But was has me "worried" is that my account says "pending" approval NOT approved. What is pending? A voucher or is this just part of a the glitch jake told us about. I email CS, but they have not given me any feedback!
I know you have said this time and time again mpbollywoodking, but where did you get your info from. That would make me feel a little less leary about waiting around, not worried about my account status.

Jake said that if you don't see your form in your Status page, that means you don't need one. You already been approved, which is why all your referrals are gone in your status page
