My friend signed up for an offer, used my name and address, but her credit card, will I be put on HOLD? Now, she put this information AT the offers site, NOT on Gratis? Like when signing up for the Video Professor, and putting her shipping information down, she did it from a completely different computer, IP, the only thing being she used my name and address? What about that? Am I safe?

Yeah, you're going to be put on hold. Just forget about doing any Gratis sites and go to another network. There are plenty so you shouldn't feel too bad. Gratis sites are harder to get refs for, anyway. At least this happened before you had all 5 refs and were ready to submit for approval.
